Initial intake evaluation (90 minutes) - $495
Therapy sessions (50 minutes) - $275
Insurance
I would be considered an out-of-network provider by most insurance companies. Services may be covered in full or in part by your health insurance or employee benefit plan. While I do not bill insurance companies directly, I can provide monthly statements that you may use to file such claims.
Please check your coverage carefully by asking the following questions:
Reduced Fee
Reduced fee services are considered on a case by case basis.
Payment
Cash or check is accepted for payment at time of service.
Cancellation Policy
If you do not attend your scheduled therapy appointment, and fail to contact my office at least 48 hours in advance, you will be required to pay the full cost of the session.
